The Beresford School District will host a Flu Immunization Clinic on Tuesday, October 27 from 4:00-7:00 p.m. in the BHS Multi-purpose Room. The clinic is free and open to anyone between the ages of 3 and 18 who has had a flu shot in the past. Six medical professionals will administer the vaccines, two will screen at the door, and two non-medical people will serve as ushers. Facial coverings/masks are mandatory for all in attendance at our flu immunization clinic.

The following link will provide you with the Influenza Vaccination Consent Form that needs to be completed prior to attending the Flu Immunization Clinic. The consent form includes a section on insurance information, but this does not need to be provided. The consent form must be filled out with a parent signature, unless the students is 18 and then they can sign their own form. https://5il.co/m1vx
